New Patients
Please call to see if we are accepting new clients. Due to staff shortages and high patient load, we may suspend accepting new clients. To ensure that we provide you and your pet the best possible care we have two forms that we ask you to fill out. If you would like to save time at our office you can download the New Patient Form and the New Client Form by clicking on the links below and fill them out prior to your arrival.
Please arrive 15-20 minutes prior to your first visit to allow us time to enter your information into our computer. If we are seeing your pet for a second opinion or as a referral, please bring a copy of the medical records and x-rays.
Please note:
For the safety of our clients and staff ALL dogs and cats must be on a leash or in a carrier at our office.
We accept cash, check, Visa, MasterCard, Discover, American Express, and CareCredit as forms of payment for services. For more information about CareCredit, please visit their website or ask one of our staff members.
